Error when saving a goal

Content (required): We have an employee who is trying to create a goal and when she clicks on the Save and Close button she gets the error in screenshot below. Has anyone seen this error before?
Employee is using this path: Me - Career and Performance - Goals - The most current Review Period and Goal Plan are populated - Click the Add Button.
