Issue with Generate Real Time Statements under View Administration Reports

We are having an issue when running the Generate Real Time Statements for our plans where the job status is successful, but the download says "The template to generate statement is not available in the template repository. Please check with your system administrator." Consequently, if we generate an individual employee statement from within the plan while Act as Proxy Manager, the same template downloads the statement successfully.

Originally, we were getting a Job Error when generating real time statements, so we deleted the Workbench Print Statements report and cleared object cache. This resolved the Job Error, so our job ran successfully, but we're now getting the template not available issue when downloading the pdf. Again, we are able to run individual statements via Proxy manager after doing this, so we know the template is working.
