  1. Have you held a review meeting with this employee? Yes/No questiona. a)IF NO: warning sign or something telling them to have the meeting. b)IF YES: date format to enter in the date of the meeting 
  2. Has the individual shown an appropriate level of knowledge and skills for the role? Text box
  3. Has the individual shown the ability to assess information, make decisions and understand consequences pertaining to the role? Text box
