Anyone had issues with Assignment information missing for transfers?

Summary:

We have had a handful of cases recently where the users have reported issues with internal transfers.

The following has been happening:

1) Certain users are getting the below error when accessing the assignment info block on the offer grid. This is not due to browser or caching.

2) Information entered on the offer grid has not transferred to the assignment when the start date has been reached. The assignment info still displays the old assignment info.
