Update of information through the year

We have each employee complete an annual conflict of interest declaration. Until now, we have used a questionnaire to ask employees to complete the relevant information. We would like to find a solution that allows employees to update the information every year, without having to create a new questionnaire and enter the same personal information one year after the other. 

Has anyone here had a similar case?
