Distribution disappearing at requisition approval

Buyer edits requisition before final approval and the distribution project/task or COA disappears

At this time, our professional buyers do approvals on the requisition side. When they edit the req, there have been times that the project/task or COA just disappears. At first we thought it was due to them having a default COA/Project, so we had them remove it. Has anyone else had this issue?
