Conditional Fast Formula Based on Application Questions as Part of Candidate Selection Process

Looking for a fast formula that checks whether the user responded to any of the following answers (codes):

Content (required):

Creating an action move formula that moves an application from one state to the other within the same phase. As of now, the formula is checking only one condition, or answer, and the fast formula is working fine and is progressing application as expected. The problem I am encountering is adding additional conditions to check, an OR statement, that looks whether the applicant responded 300000105651520 OR 300000105651522 as part of prescreening questions. If so, the condition should be satisfied. FF:
