how to add Extra flag/checkbox in Offer Approval Request

Summary:

Our customer wants to add radio box/flag to let approver choose(whether he know the applicant before) in the offer approval page(accessing from front end/from Eshare email). Could we do some customization on that pages?

And also if this page customization is not possible, any other workaround/alternative ways to acheive similar result?
