Performance Rating- Self review Rating not populating

I am trying to pull a report with Employee calculated rating after worker self evaluation task is completed. The report was running last year with rating populated , however when I am trying to run this year no data is retrieved, I think after the recent upgrade on OTBI something has changed. We have not changed any configuration on Performance document setup.

I pulled almost all rating fields, however the data is not populated. What else do I need to filter or add in the report to get the employee rating?

Any help will be highly appreciated.
