Boot Failed with Error: Warning: /dev/<VG>/root does not exist

Symptoms

The instance fails to boot.

The console, three minutes after the boot shows:

[ OK ] Started iSCSI UserSpace I/O driver.
Starting Open-iSCSI...
[ OK ] Started Open-iSCSI.
