Individual text field for segments

Summary:

Is there any way to have an individual text field in the "Edit/View activity" screen for each segment in a multiday activity?

Content (required):

Our customer asked us to provide an individual text field for each segment of a multiday activity. They would like to use this field for notes & instructions for this specific segment.

E.g. segment 1 = first visit and installation, segment 2 = do this and that, segment 3 = other activities and finish.

Is there any way to achieve this with the Visual Form Editor?
