Need Employee wise data where Checklist Response Text and Response answer in same row using OTBI

Trying to create an OTBI report from a survey which consists of both Response answer type and Text answer type questions. The report should show employee details, checklist name, status and the responses respectively. Used a pivot table - created columns based on Question texts and Response answer as Measure. But can not implement the Response text on the same row with the other response answers.

Any help on this would be appreciated.
