Action History about Approval or Reject Information

Summary:

Action time and creation time are not synchronized in po_action_history table.

Content (required):

We have create requisition with 2 levels approval.

approver A approves the requisition around 10am

till 10:30am there is no approval information in po_action_hisitory table but from requisition page,we can see user A has approved the requisition and waiting for user B to take action,may I know why?
