Orders are failing with FAULT_NAME error message

Sales Orders are failing with the unknown error fault_name error for Create Request step( Dropship Orders). Has anyone faced this error ? unable to track down the root cause of this. The configuration was not altered; it was working properly prior to this. Please see the screenshot below.
