Can I pass the value of a property via the Parameters section to a custom form ?

Summary:

I am trying to send the value of another property as the contents of a given property when invoking a custom form, e.g. ACTIVITY_NOTES = appt_number, so that when I open the form, the property ACTIVITY_NOTES there will contain the value of the appt_number for that activity.
