Send Message to Hiring Team not always sending an email

We have a template that is used when sending a message to the Hiring Team through the job requisition that is not sending a notification every time to the HR person

Content (required):

Whenever a hiring manager wants to move forward with an offer request in the hiring process, they use a template that they select after choosing the option "Send Message to Hiring Team" inside a recruit's Action list. There is one HR Recruiter that is not receiving the messages consistently. I can send the message to myself or any of the other HR Recruiters and they all receive any of the messages, but for the other HR Recruiter it is hit or miss.
