Inconsistent behaviour on 401K contribution calculation

Every eligible employee in our setup would have the Element entry with eligible percentage for Employee contribution and Employer contribution I derived based on the following logic “The company will match 100% of your contributions; dollar for dollar, up to 3% of eligible pay, plus 50% of each additional dollar greater than 3% and not more than 5% of eligible pay.” And this is coded in Fast formula.

We are seeing inconsistent calculation results for 401K ER Roth Employer contribution. When we review the payroll results from our last payroll run (Payroll name US Weekly), for few employees there is no ER Match element “TAO 401KR Roth Employer Match” even though we have active element entry with right percentage.
