Mandatory Form to be filled by hiring manager

Hiring manager needs to fill a form before moving a candidate/employee to 'Offer - to be created'
Content (required):
We have a requirement for hiring managers to fill out a form before they move the candidate/employees to 'Offer - to be created'. Based on this form, HR specialists will create the offer and selects the action reason and effective date. Can a user defined form / a form which is embedded as attached in the screenshot be made mandatory for managers?
Grateful if you could share your thoughts on this.
