23A Potential Assessment doesn't pull into Talent Review template?

Created new Potential Assessment in Questionnaire templates under Performance 360 subscriber - will not pull into Talent Review template for use

Hi All,
When attempting to configure the 23A enhancement CONFIGURE MULTIPLE POTENTIAL ASSESSMENTS FOR TALENT REVIEW; we are unable to pull our Potential Assessment Questionnaire into the Talent Review template for use. Our Potential Assessment uses the Performance 360 subscriber and is currently marked "Active".

Our Potential Assessment does not appear in the LOV for us to add to our Talent Review template.
