remove not relevant fields from contact info in the job application

When external candidate is applying for a job he must insert his contact info

We have here few fields that are not relevant at all to the Israeli candidates and we really want to remove them only for the Israeli candidates

I do see that we can change the name style in the

But it is not being reflected in the Job application contact info

Do you have any idea how can I remove this fields?
