Retrieving Enrollment Questionnaire Data

Summary:

We're configuring a learner enrollment questionnaire to obtain event registration information (e.g., name, DOB, travel information, allergens, etc.)

I created the questionnaire via the Questions --> Questionnaire Template --> Questionnaire workflow. Then I added the enrollment questionnaire to the offering. We show the enrollment form when the learner selects to enroll in the offering. Once the enrollment form is submitted, the offering is active.


We must extract the data via OTBI to retrieve the enrollment form responses.

I created an OTBI report using these dimensions. All questionnaire-related dimensions are from the Learning Record subject area, whereas the worker-related dimensions are from the Learning Management subject area. (Since this course wasn't assigned and the learners self-enroll/voluntary assignment, I am apprehensive about using the "Assigned to" related dimensions in the Learning Record subject area. Regardless, that's not my current challenge related to this question.) I've included two subject area measurements to join the two areas together to avoid getting that error message.
