Smart Form Name on submitted requisition

Hello Experts,

User is using a Smart Form to create requisition and submit for approval. Once approved or rejected, I do not have a way to see what is the name of the Smart Form used to create requisition.

Please suggest how Can i see that Smart Form Name.

Is this hidden under page? or is it available under subject area or in base table?
