Any ideas as to why Interactions and Interview Feedback Forms are not showing when saved?

Interactions and Feedback Forms Disappearing

We are hearing from our recruiting team that they are having issues with Interactions disappearing. They will enter them in on the job requisition for the candidate and then they will be gone. This is a hit or miss thing, because there are a lot of interactions that do not disappear. We are also having this same issue with interview feedback forms disappearing as well. The recruiters will receive an email notifying them that the interview feedback form is complete, but then it's not there when they go to look for it. I have done a lot of searching and
