Does anyone has a solution to add and update competencies to a person as a result of the performance review?

I'm trying to find a description of the process as defined in HCM, but this seems to be in different documents. Now we have competencies added to the job profile not to a person. Do we have to add competencies to a person profile before these are updated as a result of the performance process or how are the ratings per person modified?
