Pending worker stated they completed journey but journey is not reflecting the same?

Summary:

We have a pending hire who had the enterprise onboarding journey trigger to them. They were able to log in and complete their journey tasks. HR Admin logs-in to view the same journey and sees no tasks have been completed and now the new hire is unable to complete their tasks. This has happened to 2 new hires recently. Has anyone ever seen this or know what might be causing this ?

Enterprise Onboarding Journey - triggered via integration with action add pending worker. New hire receives message to reset password to log in. New hire logged in and completed all tasks but HR Admin shows opposite. We verbally confirmed that the new hire in fact did complete the journey tasks. Unable to understand why the tasks show as incompletely on our end and why now the new hire is getting an authentication error when trying to log in.
