How to remove duplicate fyi notifications?

We are receiving duplicate fyi notifications for change personal information approval workflow. I recently had to rebuild the rule entirely because it wasn't sending any notifications at all. Now we are getting two. I have already set the task aggregation to 'once per task' but am still receiving duplicate notifications. Please advise.
