Customer statement for ad hoc querying

I need a report (BIP or analysis), that gives me the possibility to create an ad hoc Customer Statement.

Content (required):

The seeded report "Generate Customer Report" isn't working for us, as we only need to send customer statements by request, not monthly/weekly/etc.
