Not pulling in questions from the Questionnaire into Check-In Document

HI,

I have created a Check-In template and attached the separate questionnaire for Manager and Employee.

When we create the Check-In document as Employee/Manager. On the check-In page. I am able to see the questionnaire name but not the questions inside the Questionnaire.

Attached screen shots of the configuration and questionnaire.

Can anyone please guide me if anything i have missed
