Need to have a warning message with comment and attachment when deleting a position

When attempting to delete a position ( Manage Positions), the client wants to display a warning message with the ability to add (comments and attachments)

When attempting to delete a position ( Manage Positions), the client wants to display a warning message with the ability to add (comments and attachments). I have seen this in the Oracle Demo Environment ( see the attached picture), but I am unsure how it was achieved - as of now, the warning message display without the comment and the attachment options.

Navigation: My Client Group - Workforce Structure - Manage Positions - Search for any current position - Click on Delete
