OCI: Adding iSCSI Block Volume In a LVM "/" Filesystem Causes Reboot Of OS Into Grub Prompt

Symptoms:

When trying to SSH, receive 'connection refused’ error.

Validating from console connection, it is seen that the OS went into Dracut mode 

dracut-initqueue[583]: Warning: Could not boot. 
 Starting Setup Virtual Console... 
[ OK ] Started Setup Virtual Console. 
 Starting Dracut Emergency Shell... 
Warning: /dev/mapper/ocivolume-root does not exist 
Warning: /dev/ocivolume/root does not exist 
 
Generating "/run/initramfs/rdsosreport.txt" 
 
 
Entering emergency mode. Exit the shell to continue. 
Type "journalctl" to view system logs. 
You might want to save "/run/initramfs/rdsosreport.txt" to a USB stick or /boot 
after mounting them and attach it to a bug report.
