Opt Out - Opt down form for Responsys

Hello, has anyone successfully built an Opt-Down form using Responsys forms? I'm trying to build a form that allows our customers to opt in/out of different categories of emails.

The way I envision this would be to have checkboxes on the form that when checked, and submitted, set a field in a PET table that will be used to filter emails and categories.

I'm open to suggestions, but i'm also having a hard time with the form code. Here is what I have, I was hoping someone could help me with this.
