Custome Form Link in Responsys

I have built a custom form and it states to place this link in the Campaign to link to the form:   $formlink(GradedAssignmentsOptOut)$

However, when I place that link (in the link table via click-through or direct href) I receive an error. Is there a new format to link to custom forms?
