Form to form not submitting

Hi,

We have 2 forms that we would like to work in Responsys.

The first one, simply contains an email_address_ field and a submit button.

The form actions for this are so that when the form is submitted, it proceeds to a secondary form.

The secondary form contains fields from a PET table and we can see these fields populate with relevant data from the email address as passed from the first form.

The problem is, that the second form WILL NOT submit. It refreshes the page, but won't submit any data that's entered into it.

Please help - I imagine I've done something really silly...
