form submission in to supplementary table

Hi There,

i need to store my form submission data in to supplementary table using two primary keys

i have two primary keys in my supplemental table Customer_id and form_type. Form type can be of only two values 'form1' and 'form2'

Basically there are two forms if same member which means same customer_id submits the data from two forms it has to create two seperate records in to my suppleememtal table say like below records which means i need to check two fields while submitting the data or while upadting
