How to configure Agents using specific form information in Eloqua Insite

I am trying to set up an Agent to deliver a monthly report based on a specific form. I followed the instructions I found here

Seemed pretty simple, but the Agent isn't running. The report is saying there is an error, but I don't know what the error is or how to fix it. Can someone direct me to additional instructions on how to configure reports on specific form data.
