Attempting to send different notification emails upon form submission

in Eloqua

I've been banging my head against a wall all day trying to get two different emails to send out upon a form submission. Use case is below:

A Case object is created by our service team in Salesforce. That gets synced over to Eloqua which creates a Case CDO. I've got a few of the case fields getting saved to the contact table via CDO Record Services. The main one I need is Case Record Type ID. I have also created a program that links the Case CDO to the appropriate contact and then submits a form (via the Form Submit cloud app).
