How to record all submissions to a form to a CDO without overwriting the submission?

I've created a form for use on an Eloqua LP. The customer's first and last name is auto-filled. Customers are allowed to submit the form multiple times. The problem is, the most recent submission is overwriting the previous submission in the CDO. I need the CDO to capture all submissions by the same customer. Any advice on how to achieve this?
