Report Help - Historical Status Dates by CSW Status

Report pulls event date by CSW Status Name, but creates multiple rows per candidate

Hey all,

Hoping for some help from anyone who's built a similar report trying to track the time it takes candidates to move through our steps/status.

I've got a report with case statements that pull the historical event date for each status name.  I expected this to come out with one candidate submission row including all dates, but instead each individual status name gets it's own row.

Does anyone have a solution for getting the DESIRED RESULT from the attached example?
