Checklist Questionnaire: How to retrieve responses after employee fills in the questionnaire?

Summary:

Hello, we have a requirement to create a checklist for employees who will be terminated. They'll need to fill in some forms.

So as can be seen below, for our 1st task we have added a questionnaire that the employee will fill in.

However we want to know if after the employee fills the questionnaire, can it be retrieved by the HR? for e.g a copy of it.

Please advice.
