can we create fields in screening services under requisition template ?

Summary:

Assessment configuration - We have a requirement where we want to add a new field under screening services of requisition template or in front end while creating requisition.

Content (required):

We have partner integration screening services for the assessment configuration in ORC, however we want to add an additional fields in configuration. However need to check the feasibility for this.

For eg: as per screenshot we want to add one more filed, how do we add an additional field ?
