Checklist Email Notification: Only 1 task is showing

Hello,
We have a created a checklist.
In the display settings we have checked the values as shown in the screenshot.
Also, the checklist contains 3 tasks. And for each task, on the notification and reminder tab, we have checked the notification for performer. (see the screenshots below)
However, when receiving mail, there is only 1 task showing. The other task are not showing.
Please advice.
