Unable to attach document in personal Journey tasks.

Summary:

Receiving the below mentioned error while uploading document as attachment in the task created under personal Journey.

As per the requirement, in the selected document type below attributes are marked as relevant.

'Document Name Applicable'

'Issued On Applicable'.

We have below set up configuration for selected document type. Could anyone please help me understand the significance of these attributes setups and why we are encountering the error.
