Setting up a feedback template

I have setup a new feedback questionnaire. When I create a new feedback template, the questionnaire that I have created isn't visible from the drop down list. The questionnaire is active (see attached screen shot).

Is there a scheduled process that needs to be run for the template to become available?

Any help anyone can give would be very much appreciated.
