Why does my "question" attribute show blank, when every question for that rec was in fact answered?

I have a rec that requires each submitter and approver to answer an attestation question, confirming everything looks as expected before submitting. For all of our recs, we see those questions have all been answered "yes".

If all of those questions are answered, why does the "question" attribute show nothing but blank values on the Reconciliation screen?

I have attached an example of a rec with 2 screenshots. The top screenshot shows the question attribute is blank, and the bottom screenshot shows all 3 required questions were actually answered.
