Summary:

Our employees have started to receive this failed notification shortly after they receive a "your manager has approved your goal" notification. When we check the goal in Goal management it appears as approved so the failed FYI notification is sending incorrectly. Also, this failed FYI notification does not happen in our TEST environment.
