Configuring the approval rule for Requisition using AND and OR conditions.

Content (please ensure you mask any confidential information):

Requisition approval rule is as below.

1.Condition: Requisition approval amount AND Requester department AND smart form is Blank. Action:3 groups. This rule is working as expected.

We have defined and using many smart forms. Now we want same rule and action for only one smart form.

2.We have configured rule like below and it's not working.
