What are the highlevel steps to perform for Windows OS upgrade Eg;Wi2012 to 2019 Standard

We have tried downloading the installable of win2019 and install. We have unziped the ISO file to local drive/blockvolume.Setup runs and reboot after reboot, it is stuck as the boot order is not found it . Similarly we have tried using OS Agent, but agent installation is failing. We are yet to get the reason code. Anyone tested same , please let me know.
