Succession Candidate readiness Counts are off in report

Summary:

I am trying to get accurate counts of Active Plan candidates (active assignment and active on plan) and while I'm able to pull totals accurately, the candidate counts by readiness level are off in a small number of cases. I am attaching the XML code here. Any help is appreciated!
