OCI : iSCSI Block Volumes Not Attached During Boot

Symptoms 

iSCSI block volumes are not detected during system boot. There are no active iSCSI sessions and the iSCSI service is not running (inactive):

# lsblk /dev/sdb 
lsblk: /dev/sdb: not a block device 

# iscsiadm -m node 
169.254.0.2:3260,1 iqn.2015-02.oracle.boot:uefi 
169.254.2.2:3260,4294967295 iqn.2015-12.com.oracleiaas:xxxxxxxx-xxxx-xxxx-xxxx-xxxxxxxxxxxx 

# iscsiadm -m session 
iscsiadm: No active sessions. 

# systemctl status iscsi
