When attempting to attach a file, error appears. Then when trying again, the file attaches.

I had a user report that when editing a journal entry to attach a file, on her first attempt she gets the below error:

Error: A value is required. You must enter a value.

The error appears and the file does not populate, saying that there was no file chosen. Then when she attempts again (She doesn't close the window or exit or anything. She uses the active window.), the file attaches with no issue. She also found that she is unable to delete attachments on approved journals. (shown below)

Any idea as to what is going on?
